Why you are so tired on a GLP-1

The tiredness is real, and it is on the label
If you are exhausted a few days after your injection and wondering whether you are imagining it: you are not, and it is written down.
The Wegovy label lists fatigue in 11% of patients against 5% on placebo1. The Zepbound label lists it at 5%, 6% and 7% across the three doses, against 3% on placebo2. Not enormous numbers. Real ones, and roughly double placebo.
Nobody mentions it, because it is not dangerous. It is just heavy, and heavy matters when you have people depending on you.
Where it most likely comes from
Not one thing. Probably three, stacked.
You are eating much less than you were. This is the big one and the easiest to miss. Appetite suppression is the point of the medicine, and it is entirely possible to be eating far too little without feeling hungry about it. Less fuel is less energy — that is not a side effect so much as arithmetic.
You may be drinking less too. Thirst often drops alongside appetite, and mild dehydration feels exactly like tiredness.
Your body is doing something demanding. Losing weight steadily is work, and the first weeks after each dose increase ask more of you.
What tends to help
- Eat on a schedule, not on hunger. If you wait to feel hungry you may not eat enough all day. Three small anchored meals beat two accidental ones.
- Protein at each of them, not all at dinner.
- Water where you can see it. Boring, free, and the fix for a surprising share of it.
- Expect the dip after the shot. Mine landed on days two and three. If you can arrange the heavy parts of your week around that, do.
- Do not add a dose increase to an already exhausted week. Ask to hold. That is a normal request.
When tiredness is not just tiredness
This is the part worth reading twice, because fatigue is the symptom that hides other things.
Ask about iron and ferritin if you are also losing hair, feeling breathless on stairs, or your periods are heavy. Ask about thyroid and B12. All three cause fatigue, all three are ordinary blood tests, and none of them get checked if everyone assumes it is the medicine.
And contact your clinician promptly rather than waiting if tiredness comes with not keeping fluids down, passing much less urine than usual, dizziness on standing, or a racing heart. That combination points at dehydration rather than low fuel, and the labels flag gastrointestinal reactions leading to dehydration as something to monitor kidney function for1.
The thing I want you to take away
Being tired on this medicine is common and mostly fixable with food, fluid and pacing. But "it is just the GLP-1" is also the sentence that lets an untreated iron deficiency run for a year.
